When reporting a problem, please include:

  1. Your name.
  2. Your phone, e-mail, or other means of contact.
  3. Your status within the department (staff, grad student, undergrad, etc).
  4. Your affiliation within the department (your research group, your supervisor, or your sponsor if you are a guest).
  5. As complete a description of the problem as possible.
